Tried FireFox, IE, Chrome, Safari. It aslo fails on Mac.

Same here, ive not recorded or remembered which web browsers or OSes but I select something to upload ie a pdf and you click on upload and straight away it just makes it go red.

Yeh I am trying to get a fix, it has been a problem since 4.1.11 but to varying degrees.

It does appear to upload but there is a problem with the JS which means the attachment window doesnt update. If you F5 it should show the file. Also removing that file bugs too but the system automatically removes files never used in posts.
