70-649 - Upgrade MCSE 2003 to
MCTS Windows Server 2008 Active Directory Configuration
MCTS Windows Server 2008 Infrastructure Configuration
MCTS Windows Server 2008 Applications Infrastructure Configuration
70-680 - Windows 7 Configuration
70-647 - PRO: Windows Server 2008, Enterprise Administrator

It's possible to get to MCITP Server Admin by just taking 70-649 and 70-646 though. Perhaps this is what you meant?

Windows Server 2008 Enterprise Certification Paths
I think it depends on what you already have, i did go to double check but never got round to it. My colleague only did 2 exams and he had MCSE but he may have had some other *recent* certs too which may of helped.

That PDF is your official answer though, it's what i have been referring off at work for everyone.
